This convenient 2-part, pocket-sized “booked” version makes it easy to carry and document possible observed changes in a resident’s condition. This tool prompts staff, including CNAs, dietary, rehab and environmental services, to be alert for potential change in condition indicators. It provides a simple, clear way to document and communicate changes in condition to nursing staff.
STOP AND WATCH is an acronym which arranged vertically provides a list of condition changes that staff can observe for and document by circling the letter and communicating the possible change to nursing staff.
