INTERACT (Interventions to Reduce Acute Care Transfers) is a quality improvement program that focuses on the management of acute change in resident condition. The INTERACT program includes clinical and educational tools and strategies for use in every day practice in long-term care facilities. The Early Warning “Stop and Watch" supports all care staff including CNAs, dietary and housekeeping staff in the role of observing for and identifying a potential change in a resident’s condition.
STOP AND WATCH is an acronym which arranged vertically provides a list of condition changes that staff can observe for and document by circling the letter and communicating the possible change to nursing staff.
