INTERACT (Interventions to Reduce Acute Care Transfers) is a quality improvement program that focuses on the management of acute change in resident condition. The INTERACT program includes clinical and educational tools and strategies for use in every day practice in long-term care facilities. The goal is to improve care and reduce the frequency of potentially avoidable transfers to the acute hospital. Click Here for more information.

Description

Quality improvement is a key component of the INTERACT program. On a regular basis, hospital transfers should be reviewed to see if there are opportunities to educate staff and improve care.

This QI tool is designed to help analyze hospital transfers (including ER visits, observation stay and admissions) and identify opportunities to reduce transfers that might be preventable.

Complete this tool for each, or a representative sample of, hospital transfers in order to conduct a root cause analysis and identify common reasons for transfers.

Examine trends in the data with the INTERACT QI Summary Tool (item # MP5649-4) to help focus educational and care process improvement activities.
