The INTERACT program includes clinical and educational tools and strategies for use in every day practice in long-term care facilities. DESCRIPTION
 

This tool for licensed nurses and primary care clinicians provides guidance in the form of examples of orders that may be appropriate for residents on palliative or comfort care plans who decline hospice.

Note: This tool is also found in the Advanced Care Planning Communication Guide (item # MP5654-4).
