INTERACT (Interventions to Reduce Acute Care Transfers) is a quality improvement program that focuses on the management of acute change in resident condition. The INTERACT program includes clinical and educational tools and strategies for use in every day practice in long-term care facilities. Description

The Care Paths are educational decision support tools that provide guidance on the recognition, evaluation, and management of the conditions that commonly cause hospital transfers, and offer guidance on when to notify the primary care clinician.

 

The Kit consist of ten 8 ½” x 14” posters, one for each of the Care Paths. Also includes one Care Paths Guide, a 28-page spiral bound book that also contains the ten Care Paths. Click here for more details on this guide, or to order it separately.
