Acute Care Transfer Document Checklist/Envelope - 50/pack

Description

When it has been determined that a resident/patient requires transfer to an acute care hospital, a packet of pertinent medical information should accompany them.   The Transfer Checklist Envelope includes a two-part, carbonless checklist affixed to the front which prompts for and documents contents that should accompany the resident/patient to the hospital, including medical documents that emergency room staff will need to make an appropriate evaluation. Envelope is large enough to accommodate glasses, dentures and other personal belongings.
