Return Policy
To Return a Product
Notify MED-PASS, Inc. customer service at 800-438-8884 to obtain a RA (Return Authorization) number. We cannot guarantee credit if a product is returned without a RA number.

Products Authorized For Return:
  • Single ply forms no more than 6 months after purchase date*
  • Multiple ply forms no more than 3 months after purchase date*
  • Products shipped in error by MED-PASS
  • Products damaged in shipping
  • Defective products
  • MED-PASS will pick-up defective products and products shipped in error at no charge to the customer.

    * (Revised or discontinued products may not be returned more than 30 days after purchase.)
Products Not Authorized For Return:
  • Software, Computer Diskettes, CD-ROMs, Videos, DVDs and online products
  • Customized products and special orders that meet customer approved specifications/artwork
  • Regulatory Manuals and Publications (includes checklists and management tools)
  • Reference products such as Fast Finders; OASIS Compliance Compass (Item # HC1022SP), Hospice Helper (item # HC8017SP), Change in Weight Estimator (item # CP1707SP) and other 'spinner' products; and coding references.
  • Policies and Procedures Manuals
  • Products that have been damaged during return shipping
  • Reference products that show evidence of duplication
  • Partial pads or packages of forms, or forms where the shrinkwrap has been broken
Restocking Charges
Returns that are not the responsibility of MED-PASS are subject to the following administrative fees:
  • 15% of the value of the item (but no more than $7.50/returned package.) Minimum fee of $7.50 per return transaction.
  • Customer is responsible for returning item in resaleable condition with RA number on outside of package no more than 30 days from time RA number is provided.
  • Please return the product(s) via a carrier (UPS, FedEx, Insured Mail, etc.) that can trace to provide proof of delivery. Lost packages cannot be credited.
