INTERACT (Interventions to Reduce Acute Care Transfers) is a quality improvement program that focuses on the management of acute change in resident condition. The INTERACT program includes clinical and educational tools and strategies for use in every day practice in assisted living communities. MED-PASS Receives License to Produce and Distribute INTERACT 1.0 Tools

MED-PASS is honored to have been licensed by Florida Atlantic University to produce and distribute the new INTERACT Tools.

Description

This 15 page guide, conveniently spiral bound and mounted to an easel, provides quickly accessible decision support tools to help nursing staff determine whether to report specific symptoms, signs and lab results to a primary care clinician (MD, NP and/or PA) immediately versus non-immediate.

 

Signs & Symptoms are listed alphabetically and identify which symptoms should be reported immediately or non-immediately.

 

Suggested Use:  Place next to the telephone at the nurse’s station for quick reference.

 

Various signs & symptoms also refer nurses to specific Care Paths (newly created educational decision support tools that provide guidance on the recognition, evaluation, and management of the conditions that commonly cause hospital transfers, and provide guidance on when to notify the primary care clinician).
